
Referrals
You don’t need the referral before you book.
Book the appointment first and we’ll help you ask for one. Your baby doesn’t have to wait on a fax and you won’t pay more. You win!
Two ways to get one
Pick the one that sounds like you.
The referral has to come from a midwife, family doctor, nurse practitioner, or a walk-in doctor, and it has to cover both you and your baby. We can help you ask for it, or you can diy.
Need support sooner? We’ll ask for you
If you don’t have a referral yet.
- Book the appointment you want, and pick one of the Subsidized Rate options.
- Watch your email for the Intake and Consent form and fill it in as soon as you can. We can’t ask for anything without it.
- We fax a referral form to your provider during business hours.
- Call their office to ask to be referred, let them know we are faxing them our form. We won’t make your baby wait for a fax. You are welcome to keep your consult with the IBCLC at the same rate as the OHIP copay.
Need support later? You bring your own
If your provider is happy to fill one in.
- Take one of the forms below to your midwife, family doctor, nurse practitioner, or a walk-in doctor.
- Ask them to address it to the Ontario Breastfeeding Clinic, for you and your baby both.
- Send us a copy by email or fax. If they use the online form instead, it reaches us the moment they submit it.
- Book whenever you’re ready.
Before it goes anywhere
Have these on hand.
- Your name, date of birth, health card number and version code
- Your baby’s name and date of birth, with their health card number* and version code. Please give us their due date if you are still waiting for baby.
- The name of your Referring Provider (Dr, Midwife or Nurse Practitioner) so we can help you request a referral
*A new baby doesn’t always have a plastic health card and version code yet. If yours doesn’t, please look for the health card number on a strip of paper your nurse gave you at discharge (check the diaper bag) or your midwife left you on your nightstand.

Emma and our amazing Admin Team (they’re human too:) are here to help Monday to Friday, 9am to 3pm, excluding stat holidays. Referral forms are processed during those hours.
