Before it goes
Four things make it a valid referral.
- Your patient’s name, date of birth, health card number and version code
- Their baby’s name and date of birth, with health card number and version code or EDD if prenatal
- The form addressed to the Ontario Breastfeeding Clinic, covering the parent and the baby
- Your own contact details, so the consult report reaches the right desk
After the first appointment
You get a report back.
After all initial appointments a consult report is faxed to you with our assessments, findings and initial care plan recommendations. Ontario Breastfeeding Clinic IBCLCs work in co-operation with the Ontario Breastfeeding Clinic physicians specializing in Breastfeeding Medicine.
We support pregnant, breastfeeding, chestfeeding, pumping and bottle-feeding families across Ontario, virtually, in an office, or at home.
Scope
What this referral isn’t.
This is not a referral for a general pediatrics appointment.
Continued monitoring for adequate weight gain stays with the referring or primary care provider. We work with the family on breastfeeding, milk supply and breastmilk intake.
What your patient pays
The part they’ll ask you about.
- Valid health card numbers and a copy of the completed referral for both mother and baby must be received by OBC to be eligible for OHIP-subsidized rate consults.
- Each OHIP-subsidized appointment carries a co-pay for the International Board Certified Lactation Consultant’s services, which OHIP does not cover.
- Families may choose to self-refer and book an appointment at the non-subsidized rate with the IBCLC only.

The policy your patient agrees to
Cancellation Policy
Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the practitioner’s day that could have been given to another family. As such, we require 48 hours notice (2 full business days, not including weekends or holidays) for any cancellations or changes to your appointment. Cancelled appointments with less than 2 business days notice are subject to the full visit fee.
Cancelled appointments with more than 48 hours notice (2 full business days, not including weekends or holidays) are subject to a $20+HST cancellation fee.
Rescheduling appointments with more than 48 business hours may be done at no charge. Rescheduling fees do not apply if due to Physician or LC unavailability. Refunds are subject to a $20.00+HST admin fee. In the event of a non-sufficient funds (NSF) occurrence, a fee of $20+HST will be charged.
Without a VALID referral or health card number on file, patients will not be eligible for the OHIP-subsidized rate consult with the physician and will see the IBCLC only. Patients must commit to ensuring parent and baby(ies) will be in attendance for 60 minutes during subsidized appointments and seen by the physician unless not yet born, or consent to paying the non-subsidized rate for a consult with the IBCLC only.
